Please follow your institution's HIPAA/HITECH Privacy and Security policies and procedures before transmitting any protected health information (PHI) to CBORD. Case notes should not contain identifiable patient information beyond what is minimally necessary to identify your problem; e.g., include only the MRN for a problem patient and not the patient's full name. Before you attach any files, please review and redact any PHI, such as patient names.
